Thank you for choosing our office for all your dental needs. We have created a comfortable environment where you can feel confident in the level of dental care you receive. Upon arriving for your first visit, please be prepared to fill out our insurance and health history form. A thorough dental examination will be performed at this visit, including evaluations of gum health, possible tooth decay, TMJ evaluations if necessary and any cosmetic questions addressed. An X-ray examination of your teeth and jaws is a vital part to providing a complete diagnosis and will be taken at this visit. If your first visit to our office is for a dental emergency, we will make every effort to ease the problem that day. In most cases, we will also clean your teeth on this first visit a well as provide a pre-treatment estimate of all services, explanation of any fees and discuss the time involved to complete your treatment.
All office visits are scheduled by appointment. This time is reserved especially for you. We make a sincere effort to be on time and ask the same of you. If you are unable to keep an appointment, we ask that you give us as much notice as possible.
To save time at your first visit, we have provided the initial patient form that can be printed out and completed prior to your appointment.
